Purchase tickets to win $1000 in Ball Drop $25 per ticket or (3 for $60) (6 for $100)

Raffle prizes, Goodie Bags, Spin the Game Wheel to win more prizes ($10 per team per spin) To go Box lunches will be served at the turn beginning at 10am

Plastic dividers are available for inside golf carts. Please check box in registration if you would prefer one.

As lawful consideration for my participation in this golf outing (including, e.g., playing golf, driving golf carts and using golf facilities and equipment, I hereby represent, acknowledge and agree as follows: I am not currently restricted from engaging in this activity due to my age, condition or for any other reason. I am aware my participation will cause me to be near and/or in contact with Club staff, patrons and other persons, which raises the risk to me and other of exposure to COVID-19, and I hereby agree, on behalf of myself to forever waive, release, discharge, and hold harmless Hannah’s House 119 for any and all claims, liabilities, damages, expenses, and costs, including, but not limited to, attorneys’ fees and costs arising out of or in connection with exposure to COVID-19 that may result in illness, injury, and/or death, which may now or hereafter be brought as a result of my participation in this activity. I acknowledge that I am voluntarily participating in this activity and visiting the golf course with an express understanding regarding the coronavirus pandemic and the dangers described above, and I hereby accept and assume any and all risks associated therewith.

By signing below, I acknowledge that I have read and fully understand all of the terms of this Disclaimer. I agree to follow the current CDC guidelines concerning COVID-19 Protective Measures and I am voluntarily giving up potential legal rights, including the right to sue the Released Party.
